How to Add a Form

The Forms function on a seafarer's profile page lets you add forms created in Martide to their profile. For example, you might want to include interview questions or information about their skills and experience.

To add a form, go to the main Seafarers Page and click on a name to go to that seafarer’s profile page – click the Forms button at the top of the seafarer's profile:

You will go to this page:

Here you can:

  • Click on Form under the heading Source to go to a Form that has been completed by the candidate as they progress through the Recruitment Pipeline.

  • Click Add Form to manually add a new Form. That will show you this pop-up box:

Enter a name and select a Form Template and you will go to the Form that has been saved as that template. For example:

Here you can:

  • Click the down arrow icon to use the Document Assembly Template feature.

  • Click the pen icon in the corner to complete all sections / questions in the Form.

  • Click Save and the linked embarkation will show on the seafarer's profile under the Forms tab in the Source column:
