How to Create Appraisal Forms

In Martide you can create Appraisals that let all seafarers on one of your vessels create an appraisal for any other seafarer who is on the same ship. You set the questions that you want asked.

Login to your Martide account, click on Settings under your profile icon and then on Appraisal Forms:

You will go to this page:

In our example we have already added one Appraisal Form.

Here you can:

  • Add a new Appraisal Form by clicking the blue Add Appraisal Form button.

  • Edit your existing Appraisal Forms by clicking the pen icon.

  • Delete an existing Appraisal Form by clicking the trash can icon.

  • Sort your Appraisal Forms alphabetically or numerically by clicking the arrow next to ‘Name.’

How to Create a New Appraisal Form

Click the Add Appraisal Form button at the top of the page. You will go to this page:

Here you can:

  • Enter a name for your Appraisal Form.

  • Select an Appraisal Type from the drop-down box. Note: you must have created some Appraisal Types in the system first. Here’s how to do that.)

  • Add a description, if you want to.

  • Select the vessels that you want to the Appraisals to be used on from the drop-down menu.

Now you can start adding questions to your Appraisal Form.

How to Add Questions to an Appraisal Form

Click the Add Question button under the Appraisal’s name and vessels. You will see this:

Enter the name of your question and select a type from the drop-down menu. This is how the question and answers are formatted and you have the following options:

Paragraph – the seafarer writes their answer in a text box

Multiple Choice and Multiple Choice (Multi-Select) – for both of these, you add options. With Multiple Choice questions, the seafarer can select the closest option, and with Multiple Choice (Multi-Select) questions, they can select more than one answer from a drop-down menu.

Checkboxes – you add options and the seafarer can select the closest option. This format is created in the same way as the Multiple Choice questions.

Linear Scale – these let the seafarer choose answer based on a sliding scale entered by you:

Multiple Choice Grid – you can add rows and columns to create a grid for the seafarer to add their answer to:

Checkbox Grid – you can add rows and columns to create a grid for the seafarer to check their corresponding answer. This format is created in the same way as the Multiple Choice Grid questions.

Keep adding questions and their options / sliding scales / rows and columns until you have completed your Appraisal Form and then click Save.

Your Appraisal Form will now show on the main Appraisal Forms page.

Notes About Creating Appraisal Forms

  • Creating Appraisal Forms is virtually identical to creating standard Forms. If you’ve already used this Martide feature, you’ll know exactly what you’re doing! Read more about Forms here.

  • You can use any combination of question formats in an Appraisal Form.

  • If a seafarer creates an Appraisal by mistake, you can delete it at any time.
